Taxes

We are a New York company and are responsible for collecting sales tax as applicable within New York. If you are ordering from another state you will not be paying sales tax on your purchase when you order from us – but consumers have always technically been responsible for reporting their out-of-state purchases for use tax purposes.

Order Cancellation Policies

There is no penalty for cancelling an order. We try to ship the same day we receive the order. This means that if you cancel an order, the item may very well be on its way to you already. In that case you will have to refuse delivery or incur the cost to ship it back to us.

Return Policies

We gladly accept return of unused merchandise if you are unhappy with your purchase. You, the purchaser, are responsible for shipping the item back to us and we issue a refund for the product cost minus 10% restocking charge when we receive it. We do not refund the original shipping charge. We do ask that you contact us by e-mail, phone, or snail mail to let us know that you are returning an item so that we can be prepared to process it.

We are committed to preserving the privacy of your personal health information. In fact, we are required by law to protect the privacy of your medical information and to provide you with Notice describing:

As our patient, you have important rights relating to inspecting and copying your medical information that we maintain, amending or correcting that information, obtaining an accounting of our disclosures of your medical information, requesting that we communicate with you confidentially, requesting that we restrict certain uses and disclosures of your health information, and complaining if you think your rights have been violated.
We have available a detailed Notice of Privacy Practices which fully explains your rights and our obligations under the law. We may revise our Notice from time to time. The effective date at the top of this page indicates the date of the most current Notice in effect.
You have the right to receive a copy of our most current Notice in effect. You may view and print a copy of this Notice from the "Privacy Notice" link located on the menu. If you prefer you may request a copy via e-mail, telephone or at the front desk and we will provide you with a copy.
